I am going to do a cortico-cancellous block graft in the anterior mandible. Is complete decortications of the recipient site absolutely necessary? Or can I just drill some holes at the recipient site through the cortical bone into the cancellous bone? I do not want to compromise the chance for success of the graft. When I first learned how to do block grafts, decortications was routine. Now some approaches are more conservative. What is everybody doing and what recommendations do you have.
