Is it necessary to elevate the maxillary sinus membrane during placement of implants through the floor of the sinus when there is inadequate bone height?

I have to place a dental implant in the anterior mandibular region to replace a lower central incisor which is congenitally missing.

When I was retorquing the abutment on #18 and towards the end of the torque (25N/cm2), the implant rotated with the abutment about 1/8 to 1/4 circle.

I recently attended a lecture where the idea of micromovement of the abutment / implant interface under load created a micropump of fluid movement that contributed to bone loss.

Upon flap elevation I noted that although ridge width was well maintained, there was minor vertical height loss (approx 2-3 mm dip below adjacent teeth CEJ’s). I placed the platform of the dental implant at this level.

I treatment planned a patient for a wide platform dental implant in the mandible to replace a second molar. Near the projected implant site, there was a 3mm long root tip left from a prior extraction I did 3 months prior.

In one of my first cases, shortly after placement, 3-4 threads of the implant on the buccal side became exposed. The implant has good primary stability.

Patient has received organ transplants of the kidney and pancreas. Patient also has a very low salivary output and is xerostomic due to the multiple drugs that are taken daily to support the organ transplants.
