Extract a broken implant: recommendations?

I have a patient with a broken implant in #19 area [mandibular left first molar; 36]. I have not had much experience extracting out fractured implant fixtures. What do you recommend that I use to remove the remaining implant fragment without destroying too much of the surrounding bone? I would like to remove the implant fragment and either graft or install a new implant.

Depends how much is remaining , if there are some threads internally that are usable then use the Neobiotech removal system , nice and quick , pain free and easy . If only and Tip ( rare ) then a trephine or very thin diamond bur with care . As generally a non-infective site site I would place a new Implant at the same time with peri-implant grafting to reduce the trauma to the patient of a repeat visit . Peter

I need to see a film to advise as to length, width, and distance to the nerve. As Peter stated, how much implant is usable. The Neobiotech system works well but more information is needed and any advice without a film is guarded. If you are inexperienced it would be prudent to refer.

If you cant detorque, I have great success with trephines (if correct sizing is selected - I have seen some dentists make a pigs breakfast out of the site by not using the correct trephine ie titanium shards/debris everywhere & bone destroyed) Some people advocate a brief touching of the implant with an electrosurg, and then within a week you can pull it out with college tweezers - i have NO experience with this technique but im sure there would be someone on this forum that has. Not as delicate as I would like for myself.
