Gingival Graft for dental implant placed too high?

I have a patient that had an implant in #10 position [maxillary left lateral incisor; 22] placed too apically by approximately 3 mm. Although she has a low smile line, she does not want a long tooth or pink porcelain. Are there any options to treat after the implant has been installed? Has anyone had success with a soft tissue graft to bulk up the vertical height or will any gingival grafting recede to the level of the bone? Would a graft like this create a periodontal problem ? Any help would be appreciated.

I'm fairly new to this but I believe that a soft tissue graft alone will recede after approx. 5 to 7 years, leaving your patient with an unappealing result aesthetically. I stand corrected if I am incorrect but I believe that the most stable long term solution would be to remove the implant, bone and possibly soft tissue graft then replace an implant... A bit of a drastic solution. A thorough discussion with the patient is warranted.

I agree with Sandy but a photo would be helpful to see the soft tissue defect. The bone needs to be there to support the graft and is good for thickness. Pink porcelain is not a sin it is sometimes necessary to correct a height issue vs removing everything and causing a bigger defect, I'd have to see an X-ray also to advise. Implant site preparation is very important in the esthetic zone, I routinely graft these prior to implant placement unless the bone is ideal. I also advise the patient prior to implant placement if I am concerned about an esthetic problem. Good luck.

Though solution to this is not easy as CRS says , removing implant may cause more damage & more difficult for patient to accept. 3mm apically ,do you mean as compared to adjacent teeth 21 ? As easy compromise , while putting healing abutment , take flap few mm on palatal side , reflect the flap on to buccal side of implant & then put healing abutment. This will create thick keratanised gingiva on buccal side. Let it heal & adapt well for few weeks before registering empression. Giving good emergence profile with may be castable abutment . 3mm pocket will not cause big problem. Patient should massage after giving prosthesis , use floss. Hope for best.
