Graft in the Presence of a Sinus Inflammation: Thoughts?

The patient in this case wishes to replace #14. The sinus membrane is highly inflamed, and it is at least 13 mm thick. I imagine it would be hard to tear. Yet I wonder how much this inflammation increases the chances of graft failure? Please share your thoughts and experience.

Is that inflammed membrane for sure? Seems very thick. Could it be polys or something else? I would say if the patient is unsymptomatic you can go ahead and place. I don't feel that it will have much negative impact on graft survival.

The patient is asymptomatic.

What does the OMC look like? Doesn't look like a healthy sinus, I'd get an ENT eval since if something develops after the surgery you may be blamed unless you are comfortable managing this.

First you have to do is interconsultation with ENT if thichening is not bacterial cause, can make the intervention prior antibiótics for 4 days

First the ostium must be checked . In case the ostium is patent there are no contra indications for sinus graft. If it is blocked the patient must be refered to the ENT who will decide the treatment to make it pervious.
