Implant Retained Removable Partial Denture: Recommendations for Design?

Dr. F. asks:
Can you use a free-standing single implant supported crown to retain a removable partial denture using a rest and cast metal clasp assembly? Or using just a wrought wire clasp? I have seen posts on Osseonews about using attachments like Locators or ERA for overdentures. But what if you have single implant crowns adjacent to edentulous spaces and would benefit from a design that utilized conventional clasps on these implant crowns? Are there any published studies that support to oppose the use of single implant crowns for this purpose? Any recommendations for modifications in design?

Don't do it, use the over denture design. The forces applied by the partial denture will not be in alinement along the long axis of the implant and this is not favorable to long term success.

Dr F Your most prudent choice is to reduce your lever arm by utilizing an implant level attachment instead of a crown on an implant. It reduces undesirable forces on the implant and also allows you to provide a more cosmetic prosthesis than your rest and clasp design. here are a number of attachments that you can use for this, from o-rings to locators, take your pick. dear dr you should better to go with a overdenture treatment plan, and actually I agree with dr F ,in addition I think you will make your patient happier with a well-designed overdenture using Locator. good luck

I agree with Dr.Boudet. I have accumulated a substantial number of cases similar to this one and in all of them removed the crown and replaced with a Locator or KeraTor overdenture attachment-with full success. You should always re-evaluate the "well-being" of the implant before embarking on this path, of course. All the best Danny Reshef

Implant supprot for the rpd is a good option. I can show few cases for it. They are especially great when converting Class 1 to Class 3 Kennedy by adding 2 posterior stops - use abutments with RPD frame overcasings or full contour crowns. I can provide PICS> Dan Domagala DDS MS - Prosthodontist
