LEADER Italia Implants: Discussion on an Innovative Surface Technology

LEADER ITALIA srl was founded in 1996. The company currently has two implant lines which were developed subsequent to extensive research on surface geometry. LEADER’s traditional implant line is called B.O.A.T., for Biological Organic Acid Treatment. The surface is characterized by even microcavities that stimulate fast bone formation. A newer line of implants, called the TiXos Line, is produced thru a Laser Microfused Titanium Powder manufacturing process, i.e. LST laser technique (Laser Sintered Titanium).

Basically, with the LST technique the desired implant model is produced by sintering nano particles of metal powder by a focused laser beam. The implants are computer designed and the resulting surface is characterized by intercommunicating cavities that interlock with the host bone. The cavities geometry and connections, 2 to 100 micron, are accurately selected during the project stage. This geometry allows bone penetration deep inside the implant body, creating pits and pores that are colonized by bone cells.
According to the company,

In vitro studies carried out by University of Birmingham (UK), researched conducated by Universities in Varese and Chieti (Italy) and clinical-histological trials carried out on animals and humans by University of Guarulhos, Sao Paulo (Brazil), have demonstrated the capability of these implants to accelerate bone healing thus improving fast osseointegration.

We were wondering if anyone has any comments on LEADER Italia implants, and particularly the new TiXos Line which utilizes the LST Laser technique? Specifically, do you think these implants are well suited for immediate loading?

it is about 24 years i have been practising implantology,i worked with different types of fixtures (9 different brands)unfortunately i started using leadr italy for at least 120 paitents,most of them failed.the company remains on the other side of the road when its poduct fais,no warranty,no garrany,or else,of course i have had many failures in the past(abou 1 or 2 percent )but a failure of about 25%has a meaning of something wrong in product,i,ve replaced all the fail cases with other brands and i have still 24 unused fixtures in my office which bought but i am not that stupid to use een one of them.

Dr. Behnam. How do you explain the high rate of failure in Leader system. Which loading protocol do you follow? Did you have more failures during the surgical phase or the prosthetic phase?

for 2 years now in my office, we have included at least two hundred TIXOS, at least 20%, with immediate restoration and 10-15% with immediate loading.
we lost 5 of them in a precarious situation (we knew we miss them! but we tried). I think that this implant surfaces is the only real news of the last 10 years in the field of dental osseointegration implant . I do not understand how my colleague may have had all those problems. I’d like to know his surgical protocol

I have put in more than 400 leader implants including the ones with all new Tixos surface. It is a very promising surface indeed especially for early loading even in cancellous bone. A surface like this not only provides much more surface area for better distribution of stress but also quicker healing happens on such a surface because of better fibrin retention and contact osteogenesis. In addition because of the porosity of these implants they are isoelastic so bone does not suffer from the stress-shielding effect. This is a huge advantage regarding the microtopography of this implant surface. Regarding the macrotopgraphy, well the threads of Tixos implants are not very aggresive so case selection for immediate loading if the bone quality is not good is all the more critical. Ultimately it is never the implant which fails. It is either your surgical technique for a particular case or the available bone which fails you on account of quality or quantity. Please plan your cases well, Dr.Behnam.

Dear Dr. Behnam,

we have actually data from a multicenter prospective study on more than 300 DLMF TixOs implants placed in different challenging context (posterior maxilla with low bone quality, immediate loading, and so on) by different clinicians and we are going to publish these data soon on peer-reviewed journals. We have an overall implant survival rate of more than 98% with excellent success rates. I really don’t understand how your results could be so poor. Even in University with unexperienced students placing the implants we had a survival rate >97%.. and you are supposed to be an experienced surgeon! are you sure we are talking about the same implant and the same surface? maybe you are making confusion between brands and surfaces.. these implants are available on the italian market since january 2010, consider this. if you are not making confusion,I suggest you to plan your surgeries with more accuracy

Dear Collegues , i am using Leader implants from 1996(4500 implant) and the tixos implant from september 2009 (130 patient). The success precentage is 98% for implus implants. Tixos implants have surfaces characteristics that, as Dr Sujata Goyal said in a brilliant way, make them well suitable in those cases we want to obtain a faster osteointegration and in D4 bone or in bone regeneration. We had lost very few implants only in high risk cases according to the ITI Risk Table which we use in patient’s screening.It is possible to watch a tixos implants video on youtube: implant lasermade tixos italy rapani.mov.
