Featured Lecture: “Zirconia Implants: The State of the Art”

The impact of titanium implants on our treatment options is beyond doubt. High success rates prove their reliability. However, some recent results suggest that intolerances and allergic reactions to titanium may be possible. The grayish color of titanium could be compromising in esthetically demanding regions and ceramic would be an alternative option. At present, zirconia is the only bioceramic, which provides sufficient mechanical strength, biocompatibility and esthetics.

Yttria-stabilized zirconia (Y-TZP) is progressively used in reconstructive dentistry. The clinical long-term results with single crowns and fixed dental prostheses are encouraging. The first zirconia implants were designed in the beginning of this millennium and controlled clinical studies are scarce in the literature. The mechanical strength of zirconia implants is proven in several laboratory tests. However, in a recent clinical study 3.25mm diameter implants showed a significantly higher failure rate than 4.0mm diameter implants, suggesting that the diameter of zirconia implants should not range below 4.0mm. To create an osteoconductive implant surface is a challenge with zirconia implants. Some clinical results obtained from retrospective studies show survival rates > 95% and clinical success rates > 90%. A recent prospective two-center study showed promising three-year results. It can be concluded that zirconia implants are a viable alternative to titanium implants but like with titanium implants, clinical investigations should continue.
