Sinus Elevation: Are Bone Grafts Needed?

Would an implant placed after sinus elevation without a bone graft provide a satisfactory long-term outcome? A recent study 1 evaluated the long-term clinical and radiographic outcomes of dental implant placed using osteotome sinus floor elevation (OSFE) without bone grafts.

The results:

Osteotome sinus floor elevation without bone grafts is a predictable treatment modality in the long run. But it should be used with caution when the initial bone height of the edentulous site is lower than 5 mm. The final endo-sinus bone height was found to be positively correlated to implant protrusion length measured on radiographs immediately after implant installation.

Anyone else attempt to just use an osteotome sinus elevation without grafting prior to implant placement? Results?

Bone graft studies can include histology supporting bone growth. Nonetheless is this bone truly supportive of loaded implants. Grafted bone sites have a higher failure rate than natural bne sites. DOes this mean that the grafted bone doesn't support appropriately? Stability testing for all these studies should be important but initial stability may not indicate the supportive nature of the grafted bone. The primary function of graft material, irrespective of its origin, is to maintain space for angiogenesis and osteogenesis. Any space providing mechanism would probably OK. But can we predict the support the new bone will provide. While most of the implants will succeed, how can we know what parameters to monitor to insure success? Dennis Flanagan DDS MSc
