Overkill, at least for this particular case. Maybe four (?) threads exposed on #28 implant. #30 implant seemed to be completely encased in bone. I would have placed bone graft material over #28 implant, pierced a membrane and placed the healing collar or cover screw through the membrane to affix it. Maybe a Pro-Fix screw inferiorly to further affix membrane. The surgery as performed will demand re-entry to remove the titanium. Unfortunate for the patient. Again, procedure done nicely but big time overkill.

How long where the splint left in for and was there enough bone build up when the splints were removed?

Excellent procedure. I would hope to expect to see complete coverage with new bone under the Ti mesh.

Now, now, harsh critics. While it may seem (to some) to be "big time overkill", when you have done MANY cases in the mandible where bone just "melts" away after "successful" implants, doing the graft (if you can get patients to understand) makes good sense. This is/was a very narrow ridge. Many such cases eventuate in implant failure in mandibular edentulous spaces with compromised bulk. Bulking that bone in an area of heavy occlusion was a very good--albeit more involved--maneuver.

Dear Doc J, Then why wouldn't you just do a bone graft first to increase horizontal dimension of bone then wait 6-9 months and at that point place wide implants? Just increasing bulk in that area without placing wide implants still leaves these implants in a precarious state. Big bone, narrow implants does not preclude these implants from fracturing at a later date. And I say that from personal experience. I'd be interested in seeing what happens to these implants five years from now.

Hello everybody, We had two options here: 1. To perform a classic GBR with bovine/allograft mixture and resorbable. 2. A more stable GBR with TiMesh from Igen. The staged approach was not an option because there was enough bone for simultaneously insertion. The advantage of Igen is the easy removal procedure. Because it is flexible, yet stays in place, the bone grows under but at the time of removal, we nees a small incision for that. We have to do some soft tissue enhancement anyway.
