Implants after Failed Root Canal in Esthetic Zone

This patient presented with failed root canal treatment in #7,8,9,10, [11,12,21,22], extensive decay, crown mobility and abscesses with fistulas. The case was evaluated with Scanora 3DX CBCT and designed with the @3shape Implant Studio to make a precise surgical guide with our own 3D printer @formlabs 2. The teeth and PDL were removed and the area was decontaminated with surgical curettage, ozone and normal saline 0.9%. Four Zeramex P6 ceramic dental implants by Zeramex P6 were used in this surgery with PRF, grafting, Collagen plugs, and 4-0 Nylon sutures. A temporary healing removable partial denture was used. The case was conducted under IV Sedation.

This case was well planned and executed. As long as the patient takes good care of his health, has good oral hygiene, and visits the dentist on a regular basis, it should last hopefully the rest of his life....baring an accident of serious illness.

If the last picture was the final picture I think the papillae would have been larger if implants were 4mm apart. IE.: implants in 7 and 10 position with pontics on 8 and 9. However there is nothing wrong with the way this case was done it is certainly engineered to last a long time unless the implants are closer than they appear and you get some bone die off between them.

Agreed. This would have looked cosmetically better and better long term with 2 implants in the laterals and a 4 unit bridge. Not sure why you want to place 4 here.....

Nicely done but it appears from viewing marginal gingival levels of incisors vs cuspids and posteriors that the incisors were supererupted. If that was the case, viewing radiograph of placed implants showing more room till reach nasal floor that the implants could have been placed deeper, giving the crowns running emergence subgingivally to overcome black triangles. Also, I like to deliver these prosthetics with a surgical flap with periosteal release in order to fashion gingival drape

Great planning other than the final aesthetic result is compromised. 2 implants is a better solution and they need to be deeper. Look at all the gray tissue. Personally , I think there are better implant systems on the market for the aesthetic zone. Hopefully time will work in your favor but I'm concerned if you will ever have papilla. Good luck with the case.

more aesthetic than ceramic? i thought these were made for the anterior region and thin tissue. which would you choose?

Nicely planned case. The esthetics probably wouldn't work for a 26 yr old female model but works in this case. Well done.

Very nice guided technique. I would love to see this case repeated with the Zeramex XT implants. Do you have a picture of the abutments in place without the crowns. I have placed and restored several of the P6 implants. Nice implants but a little more difficult to restore in the anterior. Thanks for posting.
