Dr. H. asks:
Last night, I had inserted a fixture in right central incisor with diameter of 4.1 mm and 11 mm length. When I incised and reflect the flap I was confronted with a protruding crestal bone in the right central area because of malalignment of the extracted central.

It was impossible to place the fixture in the palatal area, so I
inserted the fixture deep in the protruded crest. Can I solve this
problem with an angulated abutment? Also should I use cemented or screw type
crown? Should I expect future recession on the buccal aspect?

    Dear H:
    If you did that you will have to use an angulated abutment… a procera zirconia will most likely do, then you will have to do a cemented crown(procera ziconia can be luted easy).
    You will most likely have to do some bone profilyng at the moment of the second stage but if you are carefull and use ceramic abutment probably you wont get recesion, unless the platform is to deep, if so its very little that can be done, since 4.5 -5mm on top of the bone you will get a papillae only 50% of the time.I would say try a modified Langer flap at the time of the opening, so you will get a thicker gum , that helps.
