Provisional Hybrid when Converting Mandibular Overdenture to Fixed Partial?

Dr. C. asks:
I am converting a mandibular overdenture patient into a fixed partial denture. The patient has implants in the #22 and 27 areas. I am going to be placing 4 more implants. I will be making a fixed partial denture (FP 3) for the final restoration. My surgeon suggested that after he places the other implant fixtures I convert the overdenture into a provisional hybrid partial denture (fixed-detachable FPD) so the patient can get used to wearing a fixed appliance. I can do this by drilling holes in the existing overdenture and fitting it to temporary crown cylinders on the implant fixtures. Does anybody do this?

    Yes, it’s also described in many books and case reports. Just be carefull about undercut areas.

    You could do this but your surgeon has to provide a lot of information about the primary stability of the implants at the time of surgery and you should be very careful when going for immediate load on those new placed 4 implants.Even though it’s lower arch .You also have to provide a passive fit and no cantilevers .Hybrid construction has usually a metal framework that fits passively on the abutments and it;s screwretained .None the less it’s used when having buccal deficiences and extensive GBR it’s not the treatment of choice.So why don’t you go the safe way ,leave the four implants submerged use as a provisional actual denture relined with a more resilient reliner ( tissue conditioner ) and after 45 days 2 months do the uncovering and build the fixed restoration .It’s a general approach because you havent provided a lot of info about the case .Hope it’s helpul !!

    I don’t know why a patient needs to “get used to wearing a fixed”. If your slow at the office and you think you might learn something from the process go for it. I agree with the message above, make it easier on yourself and the patient. Leave the four new fixtures covered and when they are ready make your new prosthesis.

    THE ANSWER IS YES DRILL A HOLE AND USE ERA OR U BAR TO PICK UP THE COPINGS THEN ADD ACRLIC TO FILL IN THE REST OF THE HOLE AND REMOVE THE FLANGES ON DENTURE IT NEEDS TO BE CONVEX ON THE RIDGE FOR EASY CLEANING .I ACTUALLY GO TO THE SURGEONS OFFICE TO CONVERT THESE FOR ALL ON 4
