3 implants in his maxillary left posterior maxilla: Implant type?

I have a new patient who presents with 3 implants in his maxillary left posterior maxilla and 1 implant in #19 site [mandibular left first molar; 36]. He does not have any information about placed implants, and I can’t recognize them. At first I thought the upper seemed like Bicon, but it looks like #19 has an abutment screw. I don’t use Bicon so I don’t know if they have implants with screws. Can you help me to identify these implants and any other thoughts on this case?

I at first misread the post...I thought that there were three implants floating in the maxillary sinus.....but no...they are in the bone....... That bone scares me somewhat, because we know it is probably D 3 type bone, and the distal implant seems very short and sitting in minimal bone....... advice to you...... in my humbelist opinion....... try any of the popular hex type abutments after you remove the cover screws and see if you have a mate...they are all pretty much standard, but sometimes the diameters of the screws vary........ for the distal one...... try to get hold of a fixture mount that has a threaded screw....not the type that snaps off......If you contact Adin Dental Implants, formerly called Medigma Technologies in Israel, but branches in the USA and Canada, and all over the world..... they would probably have such a fixture mount...... and so withh a screw on fixture mount, there is little chance of the implant coming loose and ending up in the sinus...... so screw on the fixture mount to the distal implant, and with a ratchet in the reverse position.......take out the implant...... If you want a successful case, you will need to do a sinus lift...Summer Technique, Balloon technique, lateral wall....... graft the sinus, and place a longer implant and build a stronger ridge. With our CBCT it does not make much sense to fuss about the situation. But I would not paint too dark a picture. If anything it looks to me as if the molar-implant has a bicortical fixation, which would give it an enormous stability. Anyway 3d-diagnostics will tell. In my experience the idea of explantation does not sit very well with the patients. It might plant a seed of doubt about the whole treatment idea. Before I would remove the implant I would consider these thoughts: 1. Place another implant in the second molar region. 2d suggests more bone there. Favor the use of condensers over drills to improve bone quality. 2. Splint the implants with a temporary restoration following the old idea of adaptive loading. 3.Secondary sinus lift with lateral approach. Only helpful in case 3d shows implant exposed in sinus. What troubles me more from the prosthetic p.o.v. is the lower molar implant. Trias of unfortunate position, angulation and it's supracrestal platform position are asking for trouble.

Lower left molar could be a Neoss implant. The uppers could be Nobel Active, Neodent (copy of Nobel active with Ankylos connection)

Good luck. You have inherited a "mix n match" nightmare. The implant at #12 is Nobel Active. The other two are not common to the US market, caveat emptor.

Thank you all for help! I'm also between Nobel Active, Adin or AB. I have drivers for several Israel and EU implants, so one will be ok, I guess. :) I'm waiting for patient to come to my office and then I'll see in live. As soon as I know which implants are this, I'll measure ISQ so that I don't finish with implant in sinus as adivsed in upper post. Thx!

I want to thank everybody for help and suggestions. After several disscusions with patient he managed to receive implant stickers from surgeon who placed this implants. Upper left are Alpha Bio Tec (Israel) and lover is P1H (Italian). Everything seems to be osseointegrated and good. I took impressions for upper bridge and I'm waiting for impresion components from manufacturer for lower. The patient didn't want to remove any of implants so we will try to make the crown. Thx!
