Separate Crowns to Re-screw posts?

I recently had a new patient come into my office with these two custom abutments and crowns in hand. The implants and crowns were done two years ago (elsewhere). The implants were both AstraTeck EV. I am looking for advice on how to separate the crowns to re-screw posts. I have already attempted an ultrasonic.

Not sure if it works but I heard you might be able to send it into the lab and have them place it in the oven to try and heat separate the crown and abutment.

I am a lab owner/manager in Toronto. Yes the crowns/abutments can be placed in a porcelain furnace at a low temperature. The cement seal will break.

Perfect opportunity to make them both screw retained at this point. Highspeed with diamond and water spray through the occlusal to access the screw. The access looks pretty straight. Clean and polish the cement margins and torque back down and seal access with composite.

If you have a porcelain oven you can heat the crowns in the oven and use a low fusing glaze program and introduce the crowns very slowly keep them at the entrance for a few minutes so you don't overheat the porcelain quickly ( can crack) keep it in the oven for five to 10 min then interrupt the cycle, the cement will burn and crack, cool down slowly, then you can separate the 2 pieces very easily, you will need to polish the metal collar as it will turn dark. if you want, can make an access hole in the occlusal as well. If you don't have an oven give them to your lab to do it. I would get new screws as well just in case.

drill access holes through the occlusal and turn them into screw retained crowns.

I have encountered this problem previously. Send to your lab requesting technician fires in their furnace @ 400degrees C for approx 5 minutes to burn out the cement. If 400* is not sufficient, increase gradually, but obviously not near the ceramic firing temp!! Gently re-polish metal collar. David Broughton Dental Technician

I have had good success using a burn out oven. Place in cold oven, raise temp to 600 degrees F hold 5-10 min. Remove from oven and allow to cool. The crowns will separate easily. Bryce Dorrough DDS MS

Send them to the lab .they burn out the cement in the porcelain furnice and you gone have the post and the crown.this is my experience. I am prof. Tarek mahmoud Dean of faculty of dentistry El Alamein AAST Egypt

1-Make them a new implant crown prosthesis at regular price...for your hourly fee for time as dentist and lab fee. 2- If you feel guilty, put a healing abutment, and make them a crown prosthesis at the lab price. 3- unless the lab is heating and dismantling it for free. You are not winning, not really helping the patient with a less than superior product. Rigging it or breaking off porcelain...etc

How did the crown/abutment come all the way out. Are the implants broken?

Crack implant syndrome... Check the crack in the collar area. I will not take this case!
