Abutment and PFM crown for 4.6 X 12 mm Biohorizon implant?

I am a dentist in Sweden. Â I just had a new patient from Canada who had received a 4.6x12mm Biohorizon implant. I am not familiar with this implant system. Â I need to order an abutment and make a PFM crown. The implant platform is 4.5mm in diameter. Â How can I determine which implant components I need and how can I order them? Â I would appreciate some help from someone experienced with this system.

Get a 4.5mm prefab abutment. Then prep it for a crown prep with margins 0.5mm sub gingival take a regular impression then send the impression to lab. Biohorizon is a great implant for surgical and prosthetic applications. Maake sure when you place the final torque, torque to 30ncm then loosen and retorque to 30Ncm. Way easier than any crown prep!!!

Prepping intraorally is unecessary and a disservice to your patient. Take an implant level impression and have the lab fabricate a precise, beautiful custom abutment that follows the tissue perfectly. No cord packing, no grinding metal in the patient's mouth. The patient will love you!!

I love the Biohorizon and so will your lab. The implant is made color coded and it will be a green color. If the placement was ideal a standard abutment will suffice. You will order only the abutment and a small and a long impression screw. Place the abutment without the screw and it will locate and if the dentist was great he would have the flat side toward the buccal. Once it is snapped in place the long screw through the abutment and take a closed tray impression. Send this with an analog and the abutment with the short and long screw to the lab and leave the rest to your lab. It is that easy.

Biohorizons makes a 'transfer' analogue which can often be converted or modified by the laboratory to become the final abutment for the crown, without having to do any preparation in the patient's mouth. When ordering components, let them know if you intend to take a 'closed tray' impression versus an 'open tray' impression, so the appropriate impresssion screws are ordered, and order a 4.5 mm. analogue. As well, you need to determine if this an external hexed (less common today) vs. internal hexed implant when ordering components.Position the 'long flat' side of the transfer abutment towards the buccal surface (most surgeons placing Biohorizons orient the implant so that the long flat side of the transfer abutment faces the buccal), and secure to the implant with the impression screw, using finger pressure. Confirm full seating with a radiograph. Place wax or similar material in the top of the 'ball screw' (if a closed tray impression is selected) to prevent impression material getting in the top of the screw hole, and take your final impression. Remove the impression and replace the healing abutment. The laboratory will attach the 4.5 mm. implant analogue to the transfer abutment, reseat it into the impression, and pour the final models. Biohorizons will be able to advise your laboratory regarding if the transfer can be utilized for the final crown, and other technical information. Good luck with this case.

BioHorizon connection is Zimmer compatible. If you have Zimmer 4.5 platform impression coping, take a implant level impression, and the lab do the rest. You can have a Atlantis abutment on this also, CAD-CAM. I place both Zimmer and Bio-Horizon

Use Biohorizons 3in1 abutment and just prep intraorally, take a regular impression. I've never had any problems with this technique.

Dr. Deeter, what is your rationale for abutment "torque to 30ncm then loosen and retorque to 30Ncm" ?
