Alternatives to Nobel Implants for All On 4?

Last Updated: Nov 08, 2010

Dr. K asks:
Which implant systems are you using for the All on 4 procedure? There seems to be alternatives to using just Nobel Biocare components for the All on 4 procedure. For example, Adin Implants (Industrila Zone Alon Tavor, Israel) has now released a Nobel Biocare clone for their internal hex implants. It is a replica of the Nobel 17 & 30 degree multi-unit abutment. Has anybody tried this? Any other recommendations for alternative implant systems to use for the All on 4?

Yeah....how about deck screws and some super glue. That cheap enough?

Dear Colleague, you could try MIS 7 implants with multiunit connections , legacy 3 from implantdirect with multiunit connections, screwindirect from implantdirect with circular connection. i have used them all for all on 4 treatments and they all work well.

I have been doing the All on Four, Five, Six with Astra and Astra components. No problems. They are coming out with a more stremlined set of parts though. I prefer the Astra implant, so that"s what I use.

Perhaps you should ask yourself if you're doing a disservice to your patients to use cheap components that don't have the clinical evidence to support their usage with this cost-efficient treatment. Nobel has that.

It is my beleif that the " All On Four " is a way to avoid teh sinus of teh mental foramen. I have no problem raising the sinus. It is better to place the impants in the correct positions thatn place off axis laod on them. Place more and stop the fear of the sinus.

I have been using for 2 years in Romania the All-on-six system from Adin before they have launched the angulated abutments. It works perfect, i have a done plenty to show you great results. The "greatest" Nobel Active is in fact an Israeli implant from Alpha Bio (SFB) what we have used in Romania for more than 2 years before Nobel bought the Israeli company. So we pay for an original Israeli implant 25% from what you pay for the Swedish replica.

I think the so-called "all-on-4" technique is just an beatified terminology to describe the minimum implant to achieve an acceptable result with simplest surgical technique in an edentulous jaw. Once your patient is able to afford more implants, you can place this technique under the rug. Anyway, the concept is quite good. However, it should be reminded that there is a potential periimplantitis for the tilted implants. The unloaded side of tilted implants would be prone to infection. That's the reason why the all-on-4 technique would prefer implants longer than 8mm. Implants longer than 8mm would be less affected by torque load. Hygienic compliance of the patient would be another important factor.

I was initially very resistant to embracing the "all on Four". I started using the technique very slowly and cautiously. After several hundred arches I am a believer. I don't try to cookie cut every case and I sometimes use 5 or six implants in certain cases and situations. I do have patients come in every 3 months for hygiene. This is stressed at the consultation and is included in my consent form.
