Best method to disinfect implant surface after peri-implantitis?

I have a patient with an implant with peri-implantitis. The implant is readily accessible and I plan on laying a full thickness flap to expose the implant surface. What is the best method to disinfect the implant surface? After disinfecting the implant surface, do you recommend placing a mineralized particulate bone graft? Or do you just reposition the flap? What is your protocol for treating this condition?

You can disinfect the ailing dental implant surface with tetracycline, hydrogen peroxide, titanium 'spin' brush and laser. If you're aiming for regeneration, you can place your bone graft with membrane. If you intend on repositioning the flap apically, you should de-thread and polish the exposed implant surface.

There was a recent review on tis issue in JOMI , by Tastepe et al I think an issue or two ago where the concept of prophy jet use seemed favourable , we just use it due to our 28 year implant manufacturing know how . But as in all Dentistry many things work in different hands Peter

I have had success disinfecting the contaminated implant surface with the Waterlase laser. I have used the laser with additional antibiotic treatment and without it, with equal success. The prophy jet seems like a good alternative, but somewhat messy. If using a less effective mechanical debridement method I would definitely supplement it with chemical surface treatment with tetracycline.

Before disinfection we have some questions: -what is the nature of bone loss (horizontal or vertical)? - patient condition (hygiene, ....) ? - the degree of bone loss? - implant surface? - presence of attached gingiva? -...... If the implant is restorable, we will go to a combined treatment: - the physical treatment by laser after doing a chemical (citric acid, ..) and a mechanical debridement - antimicrobial treatment by chlohexidine and antibiotic - finally the surgical treatment Access flap surgery Bone resection (pocket reduction and implantoplasty) Bone regeneration Explantation

there was a session dedicated for periimplantitis at the Moi congress recently in frankfurt. i am afraid the above suggested methods have limitations-- the best possible solution suggested by Markus Schlee was use of normal saline..

I placed a implant 2 months ago and 3 threads are exposed due to lack of keratinize tissue. I am going to do a free gingival graft to increase or create keratinize tissue and cover the threads. what is the best way to disinfect the threads before the graft?
