Dental Implants and Sinus Lift: Guidelines?

I have treatment planned a patient for an Mini Dental Implant (MDI) in #3 area [maxillary right first molar]. I am planning on penetrating the maxillary sinus only 2-3mm. I have seen some reports where that is acceptable and others that state any entry into the sinus is below the standard of care unless there is a sinus lift procedure. Should I plan for a sinus lift in this case? In general, what are the thoughts or guidelines on penetrating the maxillary sinus with conventional or mini dental implants?

Are you talking about mini implants as in less than 3mm diameter or "short" implants as in 5mm diameter x 5 or 6mm length as they are two totally different scenarios

First, the MDI implant is discontinued. Obviously you weren't going to use a mini-implant at tooth #3(!) unless the implant was for RPD retention. I think the topic on "acceptable" sinus entry was discussed at LENGTH in other posts.

MDI implant discontinued? Yes, 3M's MDI was discontinued, but there are still many other companies out there making mini dental implants, or MDI for short.

A 3mm diameter implant with sinus perforation is asking for trouble. The bone there is probably not dense enough to resist occlusal loading. I suggest a 3.7 or 4.2mm diameter, even with an 8mm length would be better. Don't set yourself up for failure. I have a lot of experience and research with mini Implants and I love them but not there. Dennis Flanagan DDS MSc (Oral Implantology)

If you have enough length to put an implant 8 mm with at least 4.5 diameter will be good. If you do not have enough length, you have two choices: use short implant like Bicon 5x5. I used it, it is very successful or to use 8 mm with closed sinus lifting up till 3 mm is acceptable .
