Discussion Topic: Local anesthetic in implant surgery

I would like to get some opinions on the selection of local anesthetic for implant surgery. Specifically, I was wondering, which local anesthetic/vasoconstringent combination provides the longest and safest local anesthesia for implant surgery? Personally, I’ve been using Citanest, but it wears off fast. I need something to last an average of 3 hours. Any thoughts and recommendations?

Articaine infiltrations max and mandible ....... 3 hrs .......just give a bit more ... normally get bored after an hour and a half.. Peter

2% lidocaine with 1/100k epinephrine Works every time I block the mandible when I do implants

Does your local contain epinephrine? I use lidocaine or marcaine with epi. The epi is a vasoconstrictor which helps the duration if the anesthesia and hemostasis. Good anesthetic technique will help prevent any transient tachycardia.

Buccal/lingual infiltrates with 1:200k septo. Unlike sb OMS I never block the mandible. In rare instances if there is a strange branch/loop etc the block will prevent any sensation that could prevent me creating a parasthesia. Ussually lasts 1.5hours. Rarely do we need more time than that for most implant placements. Last step before patient leaves is 1.8ml of marcaine for pain control. That lasts 5+ hours.

I have heard of that so this comment is anecdotal, when I get near the bundle my patients will wince even with a block. I look for a sudden increase in bleeding also .

After injection of anestetics,make additional injections in the same place little quantity of isotinic sodium bicarbonate cooper.Thats makes a acidotic ph lover and you anestetics will work 2 time longer

For maxillary implant, I place 1 cart. of lidocaine w/2% epi directly in area (+/_ 3/4 cart in bucal;1/4 in palatal) waiting some 10 mins. later place 1 cart same way posterior and anterior to region just before beginning surgery. This is when doing one/two implants. For lower molars & premolars, mandibular block is used with an additional 1/2 to 3/4 cart beside area (bucal/lingual) for hemostasis purposes. This has worked well for me.

Articaine always works.I recommend try to do implants within a short time.
