Foundation Bone Filling Augmentation Material: A Good Choice?

Last Updated: Sep 29, 2008

Dr. F asks:
I am looking for a simple and inexpensive bone graft material. I have read about Foundation [J. Morita], which is collagen-based. It seems easy to use and inexpensive. All you do is fill up the socket. The surrounding cells and capillaries gradually infiltrate Foundation. Is this considered a bone graft material? Will this eventually produce the same kind of bone fill that I would get if I used an allograft material?

I didn't find that bone filled in when I used Foundation alone.

I haven't fully checked prices, but you should look into plain-ol' Calcium Sulphate. It looks like you can avoid placing a membrane over it and save that cost as well. There is a discussion somewhere on this site on the subject too...

Mike Stanley is right. Recently they featured a product called DentoGen, which is a calcium sulfate based bone graft. It works very well as a graft material.

Demineralized cortical powder mixed with calcium sulfate is the BEST!!! There is a special technique in how to mix. You should hear Dr. Don Callan speak around this subject. He is known for his tissue regeneration techniques...I have been using his method and getting EXTREMELY over-satisfying results!!!

dentogen, may have his place in grafting, so far I have used it with mixed results, specially when using it post extration, it leaves a socket similar to a dry socket, with a foreign body, creating a foul odor ect...

dr G, can you please describe the method you use? I am untable to attend Dr Callan lecture. I am in Africa. Any possibilty to get a lecture CD from him

Bone grafting an extraction socket is more complex than just removing the tooth and sticking a material in it. This is a bone regeneration technique. You need to use the right material to ge the right results. Extraction sockets will also heal on there own without grafting. So you need to know when use a bone grafting material and when it is not necessary. Do not go by what the manufacturer or representative tells you. If you do not know, do not graft the socket. You will end up with a dry socket like problem where nothing stays in the pocket.

Try letting the socket heal for a week or so to make sure there is a dry socket Post Op.Then put the graft material in. This way you can tell if its technique related.

Correction it should read no dry socket present

You have to decorticate, degranulate and detoxify the socket prior to use of a graft material.

All these points confirm how far we have come with bio-ceramics such as calcium sulfate. I have found that mixing calcium sulfate with demineralized bone does work well. I have since been introduced to B-TCP. This is a material discussed on this site with very positive results.

Hi Dr. F, I have been using Foundation for most of my extraction sites since it has been available in the United States. This product has been used in Japan for over 10 years. I have seen excellent results so far. For most of my other grafting procedures I use Puros. If you contact J.Morita they will provide you with studies from 1996 through 2001. I hope this helps you.

I had good results with Puros but its too expensive. Try dfdba (Miami / lifenet) or calcium sulfate or b-tcp (cerasorb). My results with Foundation were similar to that of Eric. I used Foundation a couple of times but found the results to be similar to BioOss. I no longer use graft materials from bovine origin.

To the one saying good result with the mixture of calsium sulphate+demineralized bone Do u think it is identical to a mature bone formation within 3 months? I use calsium sulphate but didn't achieve mature bone formation within 3 months

JRB

Yes I do find it identical...it is a solid platform for the implants. I have read the above statements, and I like b-tcp as well, however I can get the same results by decorticating, degranulate, and detoxify (stated by R. Hughes) and using Callan's method (mix Calcium Sulfate w/ DFDBA - 4 part bone 1 part CaSO4 - obviously not an exact science) and pay a lot less in cost than with b-tcp. You want a cloudy looking material that is well hydrated...make into a wet sand type mixture....tetracycline is added to the water which get absorbed in the bone...helps prevent collagenase. I have heard he is going to sell a DVD on his website in the future. GOOGLE his name and his website should come up.

I agree with JRB above....used this method and liked the results...also the cost factor is excellent - Regenetix - they work with a high level AATB bone bank which uses Lifenet's patented Allowash procedure...very impressed with the product and the method...

I am a person living with multiple chemical sensitivity and am looking for some help. I need to have a tooth extracted tomorrow and the oral surgeon want to use demineralized bone one he extracts the tooth in preperation for a future implant. Do you know of anyone that has MCS that has used this product? and if so, how is their health after having this product introduced into their body? Thanks so much for your help. I am Dentist from INDIA I used SYNOSS(suppose to beHA/TCP and bound by bovine collagen)to fill the cavity devoid of bone in the mandibular central incisor. Mixed sterile CASO4 .5 H2O.in a drop of Wokadine solution( povidone-iodine USP 10% w/v equivalent to available iodine 1% w/v.)and placed it over the filled cavity as barrier.I have to wait and see, how good it is going to work?. with regards gopi

Foundation works well, but you need to let it mature longer than Puros or the like. It is very easy to use and I don't think it is that expensive. Like CaSO4 one doesn't need primary closure. I like to place a cross suture over the top and place a periodontal dressing. The tissue will grow right over the Foundation. If you are looking for a very easy socket grafting product (and easy for the patient as well) you found it in Foundation, just give it a full six months to get good hard bone.
