Identify Implant from 20 Years Ago?

I have a patient who had an implant placed #30 about 20 years ago and he does not know what type of implant it is. I went to the whatimplantisthat.com website and am still unsure if I have found the right implant. I am going to attach a PLD to it and i need to order the correct parts. Any information or insight would be great. Thanks.

I wouldn't place anything on it, the top bridge is also failing, time to refer!
