Immediate Loading Using Basal Implants?

One area of dental implants that is developing more market share are Basal Implants, like Basal Osseointegrated Implants and Basal Cortical Compression screws. These can be used in areas where there is little vertical bone remaining. Are any of you using these kinds of implants and what are your experiences? How do these differ from conventional implants?

Yes... Basal Implants work very well... I have been into Basal Implantology for the past 8 years or more... having done scores of partial and full mouth rehabs with basal implants, I can confidently say that they work very well... even in "no bone" cases which in the normal case require cumbersome bone augmentation procedures. In Basal Implantology, no bone grafting is required as we take support from the basal cortical bone as the bone is least prone to resorption and infections. Most of the implants we use are smooth surface and hence risk of peri-implantitis is nearly completely eliminated. Most of the implants are placed in the least invasive keyhole manner such that the post operative recovery turns out to be smooth and fast.

The implants I use are from Dr Ihde Dental, Switzerland, and my mentor is Prof. Dr Stefan Ihde... He is the inventor of the system of "Strategic Implantology" in which he uses a judicious mix of basal and crestal implants to provide immediate loading, graftless implant solutions to all patients.... including the little or "no-bone" cases.

R basal implants widely documented? Does it has all necessary international certification like USFDA or does it available in most developed counties? Does it require intra oral welding to stabilize it?

of course the basal implants works very well with little complications like food impaction and meticulous care is necessary to maintain the prosthesis. the major advantage of basal implants is it works very well without accessary procedures like bone grafting, sinus lift and also very economical to the patients. regarding welding - so for i didn't do welding to stabilise my basal implants, if implant fails I remove and replace with bigger one

Yeah, As you said basal implants is the one of the remarkable advanced system. It is specially designed for immediate implant loading so you will get permanent teeth just within 3 days. It is possible solution for who having the less height bone and gum disease patient and it can be an option even for heavy smokers. But it is little bit expensive to regular implants and surgical procedure takes relatively longer time. But should be go for properly trained oral surgeon, other wise in untrained hands problem will undoubtedly come.

It's confusing; in which cases are the screw type used vs. the laterally placed ones? And, is there anyone using them in the US? If not, why is that?

Basal Implants are nothing different. Any longer implant which is engaging the basal bone are called Basal Implants. I use conventional axial implants which are longer greater than 16mm till 28mm in most of the cases (TTPHIL Technique) which engages the basal bone which is more mineralised and have more density. More over I utilize the advantage of the screw retained technology which is available in the crestal or axial two piece implants. This technology will help in retrievability of the prosthesis in cases of any complications like food lodgement, Gumshrinkage, Halitosis, Implant failure itself etc,This bone is more resistant to resorption. Hence I feel Basal Bicortical enagement is better than an implant tip engaging the trabecular bone which do not have enough capacity to accept the loads.
