Implant from 10 years ago: Identification?

Could someone help me to identify this implant in the mandibular right 2nd molar site. It is 5mm in diameter with an internal hex connection. It was placed more that 10 years ago.

Match it up by using the website "whatsthatimplant". They have radiographs of almost every implant out there since the 80's. I have used it a couple of times.
