Internal vs External Irrigation drills?

I have just purchased the Biohorizon implant system and found out that the drills are externally irrigated. I have always used internally irrigated drills in the past and wanted to know if anyone had any issues with success rates with the externally irrigated drills. Considering how critical bone temperature is in drilling the osteotomy site I want to be absolutely sure that I am not making any compromises in the quality of treatment. Thanks for any feedback.

Biohorizon cases work very well with external irrigation.Keep drill speed low 800-600 to not overheat bone.I made switch eight years ago, it is a nice system .Enjoy!

biohorizon or any other system has nothing to do with drilling and irrigation method internal or external irrigation will do fine once you do it correctly.

Sadly most systems are now moving away from internal irrigation due to cleaning issues .We have used them since the mid 80s and feel there are benefits in reduced post-op discomfort for the patients. Peter

Suggest you read an article by Sener.... The bottom line is that if you chill your solution and use external irrigation, you should be fine. The only thing internal irrigation does is increase the cost of the setup. Sener BC, Dergin G, Gursoy B, Kelesoglu, E, Slih I. Effects of irrigation temperature on heat control in vitro at different drilling depths. Clin.Oral Impl. Res. 2009; 20: 140-144

Dr. Carl Misch prefers external irrigation; 2500 RPM in his research keeps the bone cool. Quick "in-out" one time. Don't spend time with the drill in the osteotomy. Cooling the saline helps. You want to see pink bone in the flutes of your drill. I'll read the Sener article too. Thanks.

Since 1979 I used internally cooled trephines with 40 degree F 0.9% saline. I switched systems to external irrigation of room temp. saline and noticed no difference in integration rates.

There is no difference using internal or external irrigation or even both of them . I have been using external irrigation with Biohorizons surgical kit for more than 7 years either with internal system or one-piece implant, so far it is okay, no problem. So, go ahead !
