Maxillary Overdenture with #8 Site for Retention: Thoughts?

Dr. R. asks:

I have a patient who presents with an implant fixture in #8 site [maxillary right central incisor; 11]. I have not been able to identify this implant. I have treatment planned this patient for a maxillary overdenture and would like to use the implant in #8 site for retention. Firstly, what implant brand is this? Does the company make a Zest attachment that I could screw into it? Also, should I consider cutting down the height of the implant and making a castable custom abutment to fit a Zest attachment? Any recommendations on how to proceed?

In the maxillary central region, the placement of an implant fixture to help retain an overdenture is more of a nuisance than it is helpful. If you were talking about fixed bridgework on two or more implants that's a different story. I do not recognize that implant and I would be inclined to remove it or put it to sleep. You do not provide enough information to be able to formulate or suggest a prosthetic treatment plan.

If this is indeed a 1 peice structure and you have a supragingival post to work with you could prepare it to receive a reconstructive sphere from Rhein 83 and then position your nylon retentive cap and housing to facilitate a path of insertion with your other retainers. Spherical attachments tend to work better in the pre maxilla as there is normally a labial flare due to bone contour and locators cannot correct for the angulation as predictably.

First get rid of that implant. I am not familiar with this type but it looks very limited.

The implant isn't failing then why get rid of it? But I agree with Dr. Boudet given the limited info, that you may not need it to keep the patient PU stable. However if the patient has been used to that attachment for years, then losing it's attachment may make a difference for them. Not seeing another xray I still assume there is another over denture abutment for the implant in the 10 or 11 spot, correct? Does the patient still have this plate to look at? Has your lab been unable to Id it's denture component? Even if they can't ID specifically a good lab can create one that won't be too far off. I just can't see removing an implant because it doesn't match prosthetically. Like the good Dr. Hughes pointed out...need more info.. Is the maxillary over denture you are making a remake of existing partial or are you extracting and doing a complete denture? What is distal to that premolar? I'd implant that cuspid site to use and maybe forget about "8. Just see what lab can do and make it a secondary retentive feature to your over denture. But see I can go on and on...need more info...

Agree with not removing implant, 'orphan'it if it's not restorable, but I agree it will become an issue later if no other implants are added in mid maxilla and cuspid areas. Interesting case good luck!

Patient "without knowing" his previous treatment (or records) makes me often suspicious that something went wrong. Probably, this case is similar to it.

Why are dental records not recorded ? I feel things are far to lax in this area,with so many mistakes being made with dental implant placement
