Cemented Crown is Dislodging from Dental Implant: What Can I Do?

My cemented crown keeps dislodging from the dental implant. It normally stays intact for only 3 months every time after it is cemented. My dentist suggested a screw-retained crown, but he needs to replace the abutment, which can be traumatic. Are there any other alternative procedures I can consider here? Thanks.

Dentists commonly cement a crown on an implant with temporary cement, which is soft and if the abutment does not have the necessary retention, it can come off sooner than expected, creating a problem for the dentist and an unhappy patient. Temporary cement is used in case the abutment screw losens so the dentist won't have to drill through the crown to get access to the screw to tighten it again. With the new implant designs this rarely happens, and I like to cement my implant crowns with permanent cement so I don't have to worry about the crowns coming off at an inoportune moment. If the abutment is too short, that's another reason why it may come off sooner than normal. In cases where there is not enough room for a long enough abutment, the dentist may suggest a screw retained crown. If the abutment is adequate, ask your dentist if he would object to cementing it with permanent (hard) cement. Hope this helps, and Good luck! Carlos A. Boudet, DDS, DICOI
