All on Four vs. All on Six: Can I get same day dentures with either of these procedures?

I am 45 years old and my teeth are failing. I am told I am a candidate for the All on Four dental implant procedure, but after researching I am wondering if the all on six would be better in terms of bone loss, etc. Also, if I go with the All on 4, I am wondering if any bone must be cut out to do the all on four procedure.

Hi there… Good questions! All these should be discussed in detail with your Surgeon and Restorative Dentist. Generally speaking, bone is often reduced if the ridge is thin or if, upon smiling, any of your native gums show. This is to be certain that once restored, only the gums that are part of the All-on-4 bridge show when you speak or smile, and not the “seam” between that prosthetic gum and your real (natural) gums. Regarding 4 or 6 implants- again, the pros and cons of each need to be discussed with you prior to committing to a plan. That said, dental implants (once loaded) do help to preserve bone volume where ever they are placed, but there is good data to support the notion you do not need more than 4 well placed (and well spaced) implants to support a full fixed bridge. Typically, “All-on-4” is proposed in cases where there is a deficiency of bone volume posteriorly. That scenario limits most patients to 4 implants. And Yes, either plan should allow for an immediate fixed bridge the same day if they are placed correctly with what’s known as “high insertion torque”, meaning they go in very snugly! Best of luck!
