Placing Regular Platform Implants in Second Premolar Positions Only?

Dr. F. asks:
I have a patient missing her upper molars and second premolars. What she wants is for me to restore her smile. She wants implants in the second premolar positions only. She has natural first premolars. Is there any problem with placing regular platform implants to replace her maxillary second premolars? She has her mandibular first molars on both sides. What I am concerned about is a second premolar crown in occlusal contact with a mandibular first molar and second premolar. Will this cause an occlusal overload on the maxillary second premolar implant?

Why not place single wide-body implants on both sides and restore as first molars? Depends on her smile, if she smiles really wide then the big molar may be obvious but still look OK. If her smile doesn't extend that far back, it should work out well. Considering she does not want a second implant plcaed in each quadrant, a wide-body one will be able to withstand load a lot better anyway.

I agree with Dr R in doing a 6 and not a 5,you could even do a RP implant and support a Molar or if the arch allows a WP would be ideal.
