Psoriatic Arthritis: Contraindication for Dental Implants?

I have a patient with psoriatic arthritis. My understanding is that psoriatic arthritis consists of a systemic psoriasis of the skin with varying number of joints being affected by arthritis. Has anybody installed implants or placed bone grafts in a patient with psoriatic arthritis? What problems if any did you encounter? Is there any contraindication to implant placement in a patient who has psoriatic arthritis?

Psoriatic arthritis is commonly treated with a range of medications from non-steroidal ani-inflamatory drugs to the newest ones which are tumor-necrosis factor (TNF) inhibitors. While some of these pose no problem to dental surgery, some like the TNF inhibitors such as Enbrel, etc... can reduce the body's ability to fight infection and it is documented in the drug manufacturer's list of precautions. Even if you are familiar with these drugs and their effects, it is always prudent and recommended to consult with the attending physician and obtain clearance for the surgical procedure, even though you are not working in a hospital setting. Besides your interest in the patien't wellbeing, this documentation may save you from frovolous litigation. Good luck!

Having done implant procedures in several cases like this over the last 20 years I can recomend that you get cleared by the treating doctor, and then be extra careful with the AB therapy. Everything else should be work as usual.

Many Dentists I have visited have said 'no problem' but when I mentioned the names of Biologic medication, eg methotrexate, and others, many werenot even familiar with them, nor their side effects, the fact that psoriatic Arthritis is a disease where the body fights itself... attacks itself, hence more chance of infection and the medication itself leaves the person [patient] more likely to pick up infection. I need alot of implants, and I don't think the guarantee is there that all will be there. I am 65, I need 7 upper and 4 lower implants, and a bone graft top and lower arch. I feel alot of Irish Dentists just see the money sign... 25euro approx for the work I need done which has taken me so long to save. I realise, that implants and in particular, bone graft, will lift the face and help with anti ageing, besides giving me back being able to smile, eat food and enjoy life again.

I realize this is pretty old, but wondering if you have found any links to contraindications? I have Psoriasis and Psoriatic Arthritis, every root canal I've received, had to be removed due to infections. Now I've been thinking about implants but can't help but fear the same outcome.

Wow! Not that I’m happy to hear about your dental issues, but I’m relieved to hear it’s not just me. I also have PsA and my mouth is a nightmare. I’ve had NUMEROUS root canals, then some have failed. I have cavity after cavity and I really try to take care of my teeth! I’m now looking at implants because I feel like there is no other option. My dentist actually agrees that we can’t keep up with all the dental work. I brush I floss, but nothing helps! Now I’m wondering if implants will work or if I have to worry about the bone graft and the healing process. I have a consultation next week. Did you get implants?
