Screw loosening, breaking on PITT-EASY Implants?

These 3.25x10mm PITT-EASY implants [Sybron] were installed in 2008. They were restored in 2009 with stock cementable abutments, torqued to 25n/cm.
In 2015 the patient presented with the crown, still cemented to the abutment, in hand. After heat separating the parts, I reassembled them on the implant and torqued to 25n/cm. On 03.24.2015 the screw broke, presumably from the heat treatment. The crown and abutment were again heat separated, and a new screw used to reassemble the parts. On 05.19.2015 the patient called to say the crown had unscrewed itself again.
When removing the broken screw from inside the implant, my impression was that the threads within the implant held the screw only loosely. No rotary instruments were used to remove the screw, as it came out by picking at it counterclockwise until it came out. My best guess is that the threads in the implant will no longer grasp the screw, but I wanted to see if anyone has had the same issue. Thoughts? Thanks
