Suggestions on placing an implant in lower left first molar?

The lower left first molar was extracted 1-month prior due to bone loss. I would like to install an implant in the extraction site. I have 10mm buccolingual width and 10mm available vertical bone height. Should I wait until the extraction socket completely heals and then install the implant? Or can I install the implant now and do a bone graft at the time of implant installation? Suggestions?

No Do it now with placement and graft at the same time see our new protocol http://www.hindawi.com/journals/ijd/2015/589135/ , a good case in it , one of hundreds , preservation is the key so earlier better.. Peter

I agree with the early placement and grafting....however, I think I would have opted to place two narrow ( 3.0 - 3.25 mm x 8 mm ) implants into the edentulous area, because it is a large space. The final crowns would be two bicuspid size crowns, with less chance of developing a food trap as probably what will happen with one crown in a wide space.... as the diameter of the implant is certainly narrower than the natural root, and the mesial-distal dimension must be the same as what the natural crown was....... but in any case, it should work out well.....give the patient a tiny proxy brush to get in there after meals.

Peter. In the study they mention two beta TCP/CS products that are European. Are there any products similar or the same products distributed in the United States ?

In the US, you can buy Dentogen Calcium Sulfate and mix with any b-TCP. There are also new products coming soon that mix calcium sulfate with other synthetic products, in a syringe. We'll have a post about it soon.

Yes I, one can place an implant at this time. If you have doubts about depth control, then graft with a particulate such as OsteoGen. Overfill when grafting and use a resorbable membrane. You can safely place an easy implant at this time.

Wow! That's a lot of bone loss at the mesial of the second molar. Who here thinks an implant with threads in that distal root area of the first molar site will gain bone to cover the implant up to the platform (3mm below gingival level or at the level of CEJ on the second premolar). I'm thinking that is a tall order.

Dear All Coleagues, I really worry about the bone loss at mesial root of the second molar. In my opinion, this case is a difficult case to manage the implant vertical position. How to create bone around the distal surface of the implant with only GBR? Hope to hear the advices from experienced specialist for this situation. In this case, I prefer to do late implant placement because of the safety and predictable result.
