Telescopic crown on Prettau bridge: better solution?

I have a patient with a maxillary Prettau bridge that has a telescopic crown in area #7 (right lateral incisor). There is a screw access hole underneath the telescopic crown. The bridge is on 6 Implants and is great in every way except that the telescopic crown, which is cemented with temporary cement, comes off about twice a year and has to be recemented. Needless to say this is frustrating for the patient. Does anyone have a better solution to this which does not require fabrication of a new bridge?

I would like to see pictures of this case. Do you have any that you could post? Why not use a "permanent" cement to put the tooth back on?

If The patient is not willing to redesign and remake the prosthesis, you don't have any other alternative than cementing with a strong cement.

First figure out where the access hole would be on # 7 if you make a hole in the crown. Depending on the direction of the access hole. If the hole is towards the palatal. you are lucky. You then permanently cement the crown. Now you have a access hole that you can fill with composite or whatever you use. IF NOT, try replacing the screw so that you get an angulated screw channel and figure out - this should work. I guess Tru Abutment company will help you with that. Hope this helps. The reason for the telescoping crown is that the access hole would come through the facial if the tooth. If I permanently cement the crown then I will be unable to remove the bridge.

The prosthetic design is not clearly described. Usually a telescopic prosthesis is applied in mandibular central area where the bone is more solid because of constant implant/bone pressure upon putting on and removing a prosthesis.
