Thread Configuration on Healing Cap Different than Impression Coping: Recommendations?

Dr. P asks:
I have a patient who presented with implant fixtures installed about 12 years ago somewhere in the United States. At first I thought this was a 3.25mm 3i Osseotite parallel walled cylinder Certain. When I removed the healing cap and attempted to place the transfer impression coping I noted that the thread configuration on the healing cap was different from the thread configuration of the impression coping. Should I attempt to screw down the impression coping? Has anyone encountered this problem? Any
recommendations? Can anyone confirm that this is the implant I think it is and if not, what implant is it? Thanks.

If this fixture was place in the US about 12 years ago, it could only be a handful of implants. The internal hex configuration with the lead in bevel leads me to think this is a Sultzer/Centerpulse/Zimmer implant (all the same) and not a 3i ossteotite implant. try a zimmer transfer coping for impressioning.

This is definitely NOT a 3i Osseotite Certain, as the Certain system was not around yet 12 years ago. It was launched in 2003. Maybe a radiograph of the implant will give some insight as to what implant system it is.

quote "Should I attempt to screw down the impression coping?" /quote ABSOLUTELY NOT! Never force a round peg into a square hole (figuratively speaking, of course)! If you strip the threads, you're done for. Either wait until you know/have the correct components, or refer to someone who might.

There were only so many internal hex designs with a lead in bevel 12 years ago. Only two places to find it if it is the original - Zimmer or Implant Direct - exact same thing.
