Timeline for mini implant (2.9) loading?

There seems to be varying opinions about the timeline for loading narrow diameter implants. I have most recently placed a 2.9mm x 10mm in the #4 location [maxillary right second premolar; 15]. Achieved adequate initial primary stability. Placed the temporary crown and took it out of occlusion. I am interested in opinions about timeline for loading and placement of final crown. Also, if the timeline table would be any different for tooth #7 [maxillary right lateral incisor; 12].

Are you waiting for it to osteointegrate? If not then it doesn't matter when you load. Personally I would have expanded the alveolus grafted and placed a 4.1 standard implant but without a film or photo just a guess. I think that the type of surface on the minis determines if they wii integrate. With a wider implant you have more options with the abutment and emergence profile for hygiene. Good luck

You just opened a hornets nest, with all the doctors who never will place a small diameter implant. All implants have a place and a use. After 10 years of using then, I will tell you to use at least a 13 mm lenght MDI, if posible, then follow the protocol you described. You may take impressions and then place your final crown, at your convenience. After two months of healing, if good, all is good, as long as you do not create a traumatic occlusion. JCE

A 2.9mm implant is not really a small diameter implant, as conventional implants can now be purchased at 3mm. The integration process is therefore the same as for a conventional implant. I am happy to load these at 12 weeks, and have been doing so for the last 7 years. I see no difference in their performance to so called conventional implants. In my view small diameter implants are 1.8mm, 2.1mm and 2.4mm but even these can be loaded at 12 weeks given that they have been inserted correctly, with 35ncm of torque achieved.

The debate of minis vs conventional implants will probably never end. Mini implants that are used for crowns and bridges last longer than ones for removable prosthesis. Not sure about immediate loading protocol Some clinicians suggest. I do load them in about a month after placement and even then the contact very light. When the patients come back in for recall check ups, usually the occlusions are more tight. I nkow OCO biomedical makes 2.9mm implant. From what I know, placement protocol they have is almost same as one for their 3.0mm implant. So does that 1mm difference make it that crucial? The only thing that I have noticed over the years with minis on fixed use is sometimes you will get gingival inflammation around crowns since they are basically pontics. It's not peri-implantitis but it's gingival swelling due to inaccessiblity of a patient to certain spots to get rid of food particles.( Or a patient flat out doesn't keep it clean )..

Open up the ridge and see what you have. Chances are you can ramp down and place regular size implants. Monday I took out four minis, in an edentulous patient, that were placed in the symphysis by another doc. I placed a symphyseal blade and bilateral ramus blades and loaded with a provisional FPD. The patient was delighted.
