Torque Values to Use When Drilling Osteotomy Site?

I have taken several hands-on participation courses in implant placement. One of the areas that is not well explained is what torque values to use when drilling the osteotmy site. The motor for the drill has a setting for torque and a setting for speed in rpm’s [rotations per minute]. Also there seems to be quite a bit of variation in recommended speeds ranging from about 800 to 2,000 rpm’s. In the courses I have taken this has not been explained well. I would like to find out from users what motors you use and what torque values and speeds you use. I do not seem to be able to get a straight story from the implant companies. thanks for any help.

It varies from company to company. If you look at the manual which is provided in the box of the Implant it would show what Rpm osteotomy should be done. In most cases its like between 800 to 1200 rpm. Torque should not exceed 40 Newtons. Regarding Implant motor, PHYSIODISPENSER I use NSK, but lots of other companies are also available like WH, Seashin.

minimum torque required to achieve primary stability for the implant is 25 N cm. u may want to increase the torque up to 35 N cm if u r drilling through dense bone. Other factors to consider is the sharpness of your drills. If the drills are worn out u will need more torque in order to drill through the bone.

The torque depend on type of drilling, in case of probing or piolot drilling you can use 600 - 700 rpm speed and 35 Ncm torque, in case of twist or blade type drilling (final drill) I prefer to use 300-400 rpm speed and 25 Ncm torque. In the insertion of healing abutment or cover screw the maximum torque used= 15 Ncm.

Keep to basic principles - low levels of force when drilling, sharp drills, copious irrigation to keep it cool,dont skip drills.My opinion of torque values is that they are a bit of a myth and far too much importance is attached to it with regards to levels of integration of implants. Is it really relevant?

I think 35 Ncm of torque while insertion has importance only and only when you plan to immediate load the fixture. Secondly, drilling torque has noting to do with insertion torque, you can drill at medium torque levels of around 25 Ncm for regular threaded implants. For press fit implants like Bicon, we drill at 50 RPM and 50 Ncm of torque because at 50 RPM it is not possible to drill with 20-30 Ncm of torque especially in dense mandibular bone, and also Bicon has zero torque values while insertion, but in my hands it is the most successful implant.

One of the main purposes in cavity preparation is not to heat the bone because it loses its vitality if you exceed 43 degrees Celcius for afew minutes.Organic and inorganic components of bone are denaturated at high temperatures.Osseointegration does not take place if the cavity walls are necrosed and this necrosis is nat visible to the naked eye.Some bone is stone hard some is quite soft.Ä°f you work on hard bone please do not give yoo much pressure on the initial bur to overcome the resistance of the hard tissue instead use a small diameter round bur to perforate the compact bone.Once you overcome the resistance then you can begin osteotomy with the initial bur 700rpm speed 25Ncm torque.If you have difficulty in working you may increas the speed to 1000-1200 rpm and the torque to 35Ncm.Torque and speed values are constant physical values and they do not change what ever physiodispenser you use.You have just to calibrate the motor for your desired speed and torque.Please use copious irrigant solution while working.For this purpose I use mild cold surum physiologic Good Luck

Most implants are inserted with a speed of 800-1200 rpm depending on the hardness of the bone and the sharpness of the drills used. The speed of insertion is around 15 N and ended with 35 N using the manual torque wrench.
