Last Updated: Aug 07, 2010

Dr. S. asks:
I have a patient who has an abscessed maxillary lateral incisor (sinus present). It is beyond restoration. How long after tooth extraction implant should be placed?

you should wait at least 3 weeks after extraction

At 3 weeks clinical studies show that the success of integration is significantly lower. Time should be minimum of 3 months and ideally 6. This will produce the highest quality bone and give the greatest primary stability.

How about a px? How big is the defect? What does the buccal bone look like? intact buccal plate? tissue biotype? and smile line? Patient's general health? Depends on the above some people will extract, clean the area well and place the implant that day with a little graft material as needed. Needless to say the patient is well medicated on antibiotics. If the buccal plate is gone or health issues then graft and listen to the discuss your best attempt at a flipper or essix temp.

I routinely extract and place a dental implant in an abcessed area if I can get stability. I usually start them on antibiotics 4 days prior to extraction and implant placement and do a 10 day course and most likely will bone graft at that time as well depending on the defect. I definitely debride the area thoroughly which is key after the extraction. I have done many implants like this without problems with aesthetics, failures, infection, etc. Good luck.

I THINK THE BEST TIMMIING DEPNDS ON SEVARAL ISSUES MAINLLY THE BUCCAL BONE AND THE PERIODANTAL SITUATION .I THINK IT IS BETTER TO WAIT NOT LESS THAN THREE MONTHS AND TRY TO USE THE LONGEST IMPLANT POSSIBLE

For a type 1 socket...even with an abscess, GO FOR IT! The key is the type of socket. Make sure to debride the socket thoroughly and antibiotics. Goodluck.

There are studies that ibdicate a 98% success with placing a tooth in an involved socket.

I have very similar case ,21 site ,which I extracted 2 months ago then I've just places implant ,I've used A14 ankylos plus a course of antibiotics The key I think to debride the area after the extraction If you go fir immediate implant placement and grafting the socket ,you need to wait 6 month before you rest or it so why would you take the risk Extract ,debride, wait 3 moths, then CBCT to check the buccal plate, then implant .....

sir if bone defect is small u can go for immediate implant placement after extration with some bone graft placement.thourouly curretage the area.and if the defect is big that is whole buccal bone is losr plase bone graft and wait for six month.and at that time ie after six momth the width is less of alveolar ridge you can do ridge expancion and place implant
