Amniotic Fluid in Dentistry: Feedback?

There is a wide body of research that shows that amniotic fluid contains many growth factors and is highly enriched with multiple therapeutic agents that can potentially promote cellular response and wound healing. I have seen a few companies that offer Amniotic Fluid in dentistry (e.g. Salvin has something called AmnioSpark). It would seem that mixing amniotic fluid with graft material could help with regeneration, similar to what is being done now with liquid PRF. However, the use of human amniotic fluid would appear to be easier than prp/prf. I haven’t been able to find much research on using Amniotic Fluid in this fashion, though I did find this paper, Using PRP and human amniotic fluid combination for osteogenesis in rabbit socket preservation. The study concluded that PRP + human amniotic fluid (PRHA) and human amniotic fluid alone (HAF) both seemed useful for enhancing bone formation. But, since there was no difference between HAF and PRHA, “it seems beneficial to use HAF due to its simplicity of application.” I’m wondering if anyone here is using human amniotic fluid for regenerative applications, and if so, what has been your experience? How exactly are you using it? Thanks

Interesting that you are describing a use I have been implementing since I discovered the HAF from Salvin. I had been using Gem21s in a similar fashion for numerous years prior to the “disappearance “ of Gem and before it was released again by a new company. I currently soak BioOss collagen plugs in the Amniospark liquid prior to either bone grafting or after lasing deep perio pockets for which I intend the combo to be utilized as a source of regeneration of the periodontal apparatus. I have noted quick recovery of tissues with the process and I have been able to place implants into regenerated bone in previous guarded situations and gorgeous pocket closure and limited shrinkage around previous infected periodontal pockets. And the Amniospark needs no refrigeration!

I as well have tried amniospark. I’ve used in in soft and hard tissues grafting situations. I can’t say I have seen any increased healing or more rapid regeneration.

There are many things offered to dentists that I wonder about the legality of. These products are in the category that would require FDA approval but somehow they come one after the other with very little research to offer. Only in America can one take advantage of a system that is to week to monitor what is being offered to the public. The third world influencing America not the other way around.

Incorrect. These products are currently considered human tissue products, i.e. HCT/P, and as such they fall under the supervision/regulation of the CBER and absolutely do not need FDA approval. For more information, please read: https://www.fda.gov/vaccines-blood-biologics/tissue-tissue-products As far as research, there is tons of research on Pubmed about amnion products. These are already used extensively in orthopedics, and ocular surgery, and the results have been very good.

FDA is starting to refine this position regarding amnion products - with the market leader now being forced into doing a BLA (and corresponding clinical trials/RCTs) to support claims in the medical arena. Will be interesting if they meet the burden of proof that PMA products have to meet (like Emdogain and GEM 21S). I hear amnio fluid products may be pulled from the market soon until claims are substantiated.

AmnioMTM (multi potential tissue matrix) injectable is already available via Alvelogro company. I’ve used it for years.

Curious to know how the company can claim bioactivity given the product is eBeam terminally sterilized. These typed of products are facing incredible scrutiny on the medical side and have no reimbursement going forward, as there is zero randomized controlled trials indicating safety or real efficacy and the payers are not covering. The rhPDGF products, on the other hand are being reimbursed on the medical side (same product as GEM 21S) due to the body of evidence in RCTs available.

If you're looking for healing factors why not use prp and prf . Once you get the hang of it it's relatively easy and the material is free. There are also different products you can make like prf membrane s and reconstituting donor bone we with pro then adding the ppp layer from the prf tube 4o make sticky bone which solidifies

I do like the idea of using the patient ‘s blood for prf membranes and sticky bone and that is coming shortly. I must admit some apprehension as to becoming a phlebotomist.
