Can micro threads reduce marginal bone loss around dental implants?

Can microthreads in the crestal portion of an implant reduce the amount of marginal bone loss (MBL) around implants? A recent review investigated this issue by comparing studies for marginal bone loss around dental implants with and without microthreads in the neck.
The conclusion:

Meta-analysis showed that microthread design in the implant neck can reduce the amount of marginal bone loss; however, randomized clinical trials included in the review were few and the difference was small. In clinical practice, an implant with a roughened surface and microthreaded neck could be selected to maintain bone level.

I have restored many of both type of implants over the years and think there are too many variables at this point. I see many implants with micro threads that are placed with the threads above the crestal bone, clearly showing that whoever placed the fixture has a poor understanding of just what they are placing in the mouth. Add to this poorly fitting components and restorations and the picture is further complicated. I have patients with Braenemark implants that I restored 30 years ago that look like the day they were placed. We have an obligation to be critical and use only evidence based procedures and methods on our patients.
