Half implant placed at 1000 rpm with irrigation?

Last Updated: Jul 04, 2016

I placed an implant but made an error in the initial insertion speed. I had mistakenly left the motor on drilling mode and had inserted the implant halfway down at 1000rpm with maximum irrigation. I realized my error and stopped. I inserted the implant to its proper position manually with a ratchet. I achieved acceptable primary stability. Should I be concerned about inserting the implant to half its length using 1000rpm? Will this compromise its osseointegration? What do you recommend?

FWIW, I think what's done is done and there is no reason to do anything other than wait and see. This is not an automatic failure by any means. (BTW, I've made that same mistake with a lower first molar implant. I just buried it, which was the plan anyway, and it turned out fine.)

Man that stinks, happened to me as well once. That implant is still fine and I went much further than halfway. Document it and monitor you'll be fine. Quite a pucker moment heh?

I too did this at 800 rpm, was startling to say the least, went all the way to depth. It did fine

That is OK. Any heat is absorbed by the implant and handpiece. I have had this happen as well, no loss of the implant. Allow it to osseointegrate. Dennis Flanagan DDS MSc

I think this happened to many of us but i am surprised to in literature there was no study with implant insertion speed with heat generation and success but i found one study suggest that high speen implant insertion leads to high initial ISQ which suggest hogh initial stability an area to research.... Waiting for more comments from very experienced persons.

The initial half of the insertion, assuming this is a tapered implant, is a very low friction seating. The real torque and friction occurs in the last half of insertion so you should have no problem with heat generated bone damage.
