Implant Healing Caps Exposed: Recommendations?

I had a patient come here last week to the regular x-ray (one month after implant surgery on teeth 35, 36 and 46). Two weeks after the surgery the implant healing caps became exposed and I advised the patient to use chlorhexidine gel over the implants. Well, last week I took an x-ray and the patient has a vertical defect on one of the implants (tooth 35) as shown in the picture. The other 2 implants are OK. I have to mention that the patient has good hygiene. Any thoughts on this?
