Implant Mobile: Should I Give It Another Chance?

Dr. M. asks:
I have a new patient who has come to me to have an existing implant restored in the area of #4 [maxillary right second premolar]. The only information that I have is that the implant was placed approximately 8 months prior. I believe it was an immediate implant following the extraction of an abscessed tooth. On clinical examination the implant is mobile and bone loss can be seen on radiograph. The patient says that the implant has been asymptomatic. The patient does not want to see a specialist, but just wants the implant removed and a bridge done to replace the missing tooth. Is there anything I should do to give this implant another chance? If not, is there anything specific to be done when removing it when there is minimal bony integration?

Pain and worst of all mobility around an implant is a death sentence Remove it ...no more need be said

Hi firstly u need to know ur compactibility to manage complications if u can treat this check clinically expose the periostium see for bone defect do degranulation of deceased tissue treat with bone augmention and use membrane and make sure u have primary closure see pat is ready for implant removal so u have nting to loose and it surely works

order a ct scan to verify the possibility to place immediat implants and remove it as soon as possible

The mobile implant must be removed. Discuss bone grafting with your patient. Often they "just want to be done with it" at this point, but if the buccal wall is gone they will lose quite a bit of B-L width. Poor cosmetics and food entrapment may be in their future if this aspect is ignored.

Remove the implant and currette the fibrous tissue. Discuss bone augmentation to enhance the aesthetics for a bridge or even give a new future implant site
