Occlusal Changes near Implants?

Last Updated: May 30, 2017

Has anybody noticed that the occlusion of teeth adjacent to the implant restoration changes? I have noticed this change before and after the final implant restoration is delivered. What is this due to?

The mesial tooth can drift to the mesial opening the contact. So consideration must be made for such an event.

Promixal contact gets open up and food impaction. Distal tooth tends to migrate distally as implant in first molar and 2 nd molar migrate distally. Sometimes even proximal contact ceramic chip off due to unsupported by metal frame and all this leads to food impaction.

What a loaded question! Teeth move. (Periodontal ligament). Implants don't. (Ankylosed/Osseointegrated). All implants should be planned tooth down to the entire occlusion. The question is what is an ideal occlusion for a stable tooth and stable dentition. I subscribe to the idea of Occlusal Disease (OD). Other practitioners refute it citing literature to the contrary. We all know that there are so many variables to control in studies that at the end of the day what works best in your hands based on literature, experience and circumstances is best for the patient. In my humble opinion, if you plan tooth down, satisfy the three golden rules of OD and place implants by computer guidance, your patient will be in good shape. I recommend provisionalization for a period of time (arguable length) before final restoration. For more on OD see: www.ruizdentalseminars.com Thanks for the loaded question to begin a discussion.
