Crown and Abutment are Loose: What are my options?

I just got my 2 abutments and crowns put on my dental implants. One is fine and working well, but the other loosened several hours after I left the office. My dentist says that he can’t take the crown off as it will possibly mess up the abutment and implant. Do I have to live with this loose tooth until it comes off the abutment. That could take months or years. What can be done? The dentist says that gum tissue probably got in the way and retracked and made it loose.

It is your dentist's responsibility tocorrect the problem, which is a loose crown on an implant. The correct thing to do is to remove the crown and there are several ways to do it. I prefer to drill a hole through the crown to get access to the abutment screw and remove it to inspect it. If the crown can be separated from the abutment without damage to the abutment then replace the abutment and tighten the screw to the correct torque setting with a torque wrench, then temporarily recement the crown untill a new crown is made. Another alternative is to drill the access hole through the crown and tighten the screw again to the proper torque. then place a tooth colored filling over the access hole in the crown. Sometimes excessive forces such as lateral forces can cause premature loosening of the screws so your dentist needs to check the bite after cementing the crown and adjust the bite to avoid those lateral forces on the crowns. You do not have to live with the loose tooth and your dentist needs to address this as soon as possible. Good luck! Dr. Carlos Boudet

The situation you are describing requires that your dentist tighten the loose crown. If that is not done, further problems will occur including screw breakage and possible implant failure. In my practice, I would drill through the crown and tighten the abutment. I would reccomend that this is done ASAP as it will worsen rapidly.Dr. Michael Tischler
