Reconnect Fixed Partial Denture: Any Ideas?

Dr. L asks:

I have a new patient who presented with an implant supported fixed partial denture in the lower right quadrant. The fixed partial denture was retained with two small prosthetic screws. The fixed partial denture has been dislodged and both prosthetic screws have fractured. Can anyone identify these implant fixtures or prosthetic components? I need to replace the prosthetic screws. Any other ideas on how I could reconnect the fixed partial denture? Is there some way I could use Locator attachments?

Speak to your lab they might be able to help. I found a website called whatimplantisthat.com very helpful in similar case. Locators are for fixed removable prosth. so a fixed option wouldn't work.

From your Radiographs, it looks like you have the option to reuse the fixed-detachable abutments and build the new prosthesis on top, or remove them and go implant level with your impression. I do not recognize the implants other than they are press-fit cylinders withouth indexing capabilities. You also need to consider how you are going to connect the new prosthesis to the natural teeth, if they are mobile it is not a good idea.

Sustain brand implants were the vanguard product of Lifecore Biomedical in the late 1980s. They are press fit, coated very well with HA, platform has the bevel you see on abutment. If the prosthetic screws are the only problem, call Keystone Dental, formerly Lifecore Biomedical. Let them know you have the internal bevel sustain implant and the prosthetic abutment and you only need the Prosthetic screw. If they don't have them, call Attachments International and ask for Darrin who will know this product well. Quite a nice case considering it was most likely placed before 1994 which is the time the external hex "restore" implant came on the market. Hope this helps Leonard

ps. they will ask for implant diameter: these came in 3.3, 4.0 and 4.7. These on xray are 4.7. The screw is the same for 4.0 and 4.7. Good historical implant, one of the first wide bodies.

you may want to consider placing another implant in the #28 position to get rid of the anterior cantilever. It probably rested on the natural teeth but still there is a cantilever present that torques the small retaining screws. And no, locators shouldn't even be in the thought process for retaining a fixed bridge.

In response to the Locator question: The failure of these abutments after 20 years allows you the opportunity to re-evaluate treatment needs. Locator attachments would allow the ability to restore the mandibular arch with an implant supported removable partial denture. That may be appropriate if teeth are now missing bilaterally. If the rest of the arch is stable, and the bridge is in good condition, simply order new abutments from Attachments International. Hopefully identical replacements are available and you can re-cement the bridge. Happy patient. Happy doctor.

Call Darwin @ Attachments International 800 999 3003 send him e-mail photos and he will be able to help you. You will have to get 2 new screws and than screw the partial dent. back in exact position and pull it in impression remove impression and than unscrew the screws send the imp. and the partial denture both pieces to your lab they will than have to get implant replicas to screw into the both pieces of partial dent. to make a model. Know they will have to make a jig of the existing set up of teeth to replicate the placement, bite etc.. have them remove all of the acrylic and than you can have the frame laser welded re -do acrylic to completion for final delivery.

It looks like this prosthesis is abutted to natural teeth and implants. I would change the abutments and place copings on the natural abutments and cement the case. Remember the little screw is the weakest link! Also use an implant protected occlusion!

These are old Sustain implants from Lifecore which is now owned by Keystone Dental. They may be able to supply the parts needed.

I need 3- 4.0 mm Sustain fixed detachable abutments 4x6 mm. W/ 3 Prosthetic screws. Tried keystone ,out of stock. any help to locate these items would be appreciated.
